National Diabetes Audit (NDA): Helping to improve diabetes care

BLOOD PRESSURE APPOINTMENTS

Please remember to arrive at least 5 minutes before your appointment to give your heart rate a chance to steady. Please wear loose clothing and refrain from Caffeine and Nicotine for at least 30 minutes prior to your Blood Pressure appointment.

 

All Dr. Bunney's patients have been transferred to his job-share partner Dr. Jackie Tolhurst. She has increased her number of surgeries per week. We are currently interviewing for his replacement at the moment and awaiting appointment. If any of Dr. Bunney's patients wish to change their doctor please speak to a member of reception. Dr. Oliver Hassall is taking on new patients.

Mental Health

North Devon Depression & Anxiety Service

The North Devon Depression & Anxiety Service provides psychological therapy for people who are over 18 years of age and who are feeling stressed, anxious, low in mood or depressed.

Your GP can refer you or you can refer yourself by phoning 01271 335041 and the service will let your GP know.  Outside of office hours there is an answerphone - if you leave your name and contact details, you will receive a call back.

PHQ-9 Questionnaire

PHQ9 Questionnaire You may have been asked by your GP to complete a PHQ9 form. This is a tool used by doctors to assess levels of depression and response to treatment.

To complete the questionnaire, please click on the link below and follow the instructions on screen.  Once complete, this information will be sent to the Practice, and reviewed by your GP.
